Are you an experienced Advance Practice Registered Nurse (APRN) with a heart for patient care and a mind for innovation? Do you love the idea of blending high-tech solutions with meaningful patient relationships? If so, we need YOU!

We're looking for APRNs licensed in Mississippi (MS) & Florida (FL) to join our team and help us deliver top-tier remote care!

What You'll Do (and Love!):

• Conduct Initiating Visits for Care Management Services that prioritize patient-centered care.
• Create, implement, and oversee care plans for patients with chronic illnesses.
• Diagnose & treat non-emergency conditions with efficiency and compassion.
• Manage treatment plans for remotely monitored patients.
• Partner with our Nurse Coaches to prevent unnecessary ER visits.
• Provide expert care for high-risk and rising-risk patients in collaboration with PCPs and specialists.
• Refer patients to specialists when needed.
• Foster strong patient relationships through education, coaching, and proactive care.
• Particiapte in team huddles with nursing and support staff.
• Be the go-to medical resource for Nurse Coaches.

What You Bring to the Table: 

Required Experience:

• APRN (Nurse Practitioner) with 3+ years of experience in outpatient or primary care. 
• Focus in Family, Adult, or Gerontology.
• National Certification is required.
• Active & Unencumbered RN & APRN License in Mississippi (MS) & Florida (FL).
• Ability to adapt and learn new technologies quickly with proficiency in EMRs and computer systems.
• High-speed internet.
• An entrepreneurial, can-do attitud e - we love forward-thnkers!
